Thank you for considering employment with the Lexington Lansing Hotel. In considering your application, we may confirm the information presented, which may include ordering a motor vehicle driving record, a consumer report, educational records, a criminal conviction report, and past present employment records. Your signature on this application authorizes us (1) to conduct the above investigations, (2) to order a consumer report, (3) to authorize educational institutions you have attended to release your records to the Lexington Lansing Hotel, or it's authorized representatives, and (4) acknowledges receipt of the required disclosures.

The Michigan Handicapper's Civil Rights Act requires employers to accommodate handicappers under certain circumstances for purposes of employment. A handicapper may allege a violation against an employer regarding a failure to accommodate under the Act only if the handicapper notifies the employer in writing of the need for accommodation within 182 days after the date the handicapper knew or reasonably should have known that an accommodation was needed. I understand I have a right to terminate my employment and compensation at any time and for any reason and the Lexington Lansing Hotel retains the same right to terminate my employment and compensation at any time and for any reason.
AUTHORIZATION AND RELEASE FOR PROVIDING EMPLOYMENT INFORMATION
I authorize educational Institutions I have attended, and my past and present employers, to release my records to the Lexington Lansing Hotel with any or all information regarding my employment and the nature and reason for my separation of employment. I hereby release, discharge and hold harmless, the above and its affiliates, directors, officers, employees, agents and representatives from any and all claims or liability of every nature and kind growing out of or in anywise pertaining to the furnishing of such information.
